Our process begins with an initial conversation over the telephone. We focus on  current living challenges, care concerns and  how we might help. The next step in the process is:

Initial Consultation

During the consultation, we will have a thorough discussion about the aging adult’s medical condition, mobility, habits, preferences, lifestyle, and other specifics, learning about your family’s unique situation.

In-Home Assessment

If we decide to move forward, the next step is to conduct an in-home assessment of the aging adult’s current living situation. That may include:

  • medical conditions and medications
  • mental and emotional assessment
  • assessing ability to complete normal tasks of living, such as dressing, bathing or  cooking
  • evaluating physical space to ensure conditions support the older adult’s safety and well-being
  • review financial or legal needs

Make Recommendations

After our in-home assessment, we’ll provide recommendations and develop a care plan:

  • implements changes to the living environment
  • refers to resources such as in-home care, elder law and financial matters
  • suggests resources to enhance safety and quality of life
  • coordinates medical appointments with appropriate specialists

Administer and Monitor

Once a care plan is in place, we work with you and your family to implement the plan. This may include:

  • attending appointments with clients and advocating on their behalf
  • providing health care provider referrals
  • helping evaluate and select senior living communities
  • monitoring progress to ensure the plan is properly implemented
  • responding in times of crisis

Additionally, we schedule regular status appointments with appointed family members to review and ensure that all care objectives are being met as outlined in the care plan.
