Grace Wysocki, MSW, C-LCSW

Professional Care Manager

Raised in a small Michigan town, community focus and service were the hallmark of family life. Grace learned caring skills early when she actively supported aging grandparents.  She broadened her world and knowledge with bachelor’s and master’s degrees in social work as a baseline to a career in healthcare.
Starting with a senior living community, she later honed her medical social work and care management skills in acute care facilities which included primary care, skilled nursing and trauma units.  Grace gained additional experience working in a home care agency, learning the protocols of care assessment and screening tools, coordinating services with healthcare partners and client care planning.  Pursuing counseling studies in end-of-life care and grief and loss, she deepened her ability to support older adults and families in the aging journey.
Grace received her bachelor’s degree in social work from Michigan State University and a master’s degree in social work from University of Denver. She continues to pursue interests in neurology and legal issues relating to healthcare, as well as professional organizations such as the National Association of Social Work and American Heart Association.   Alongside her career, Grace has made time for active participation in philanthropic organizations including Volunteers of America and community organizations.
